Please note that you will be scheduled with the first available physician (Shah or Roelke). If you have a physician preference please indicate it in the appointment area. Finally, if you are a new patient, or have changes to your insurance, please include your insurance card info (group number and subscriber ID) in the ‘reason for visit’ field.

**Please also note that you cannot directly book for procedures. All procedures (surgeries, cosmetic treatments) need to be booked on the phone or by email so we can ensure that the appropriate amount of time is blocked for your appointment. The scheduling tool is for normal office visits for new and established patients.
