Arrive early

Please arrive 15 minutes prior to your appointment time to give adequate time for check-in. Although we don’t use many forms, we still have a few pages for you sign.

Bring with you:

• A valid photo ID.
• Your insurance card(s), both for your primary and any secondary insurance we can bill.
• If you are being seen for a worker’s compensation or personal injury matter, please bring claim and billing information along to your appointment.
• A method of payment for your co-pay. Clarus Dermatology can accept credit card, cash, or check payment for the co-pay amount, which is due at the time of your visit.
• A complete list of your current medications, or the medications themselves.
